Dr. Meek is able to prepare letters and complete forms for clients, who may need documentation of treatment for a variety of reasons.  Clients must sign a Release of Information, in order to have any treatment information released  verbally or in writing.  Clients are responsible for providing all necessary information paperwork related to requests for such documentation.
